Have you heard of Connected Community Schools LINK support services? This part of CCS is intended to connect families with support services outside of school as needed. LINK offers a broad range of services, including assistance with clothing, medical and dental care, mental health, food insecurity, substance abuse, housing, childcare, holiday aid, education advocacy, and more. CCS site coordinators provide short-term assistance to families while also providing them with resources, referrals, and care management connections to achieve long-term success. Families can get these services in two ways:
LINK Team Referral:
Each building in Canastota has its own LINK team made up of the school principal, nurse, psychologist, counseling team, CCS site coordinator, and CCS project manager. This team meets monthly to discuss student concerns and can establish a new LINK at any point during the school year. If your child or family requires extra support outside of school, this is a great way to receive it.
Community LINK:
This is an opportunity for you to receive assistance directly from your local CCS team! To do this, go to https://connectedcommunityschools.org/self-referral/, complete the form, and keep your phone on the following day to answer your local site coordinator's call.
